________________________________has my permission to travel off-campus in his/her car, or to ride with another media student to a media assignment as part of a classroom related activity of a Broadcast Journalism class at Hillcrest High School.

The following section is to be filled out, signed, and notorized, for student to be eligible for off-campus trips to journalism events and conventions.  Additional trips may include a Spring Break event that will be planned for the Broadcast Journalism II classes in the second semester.

I hereby authorize my child to travel to santioned events with the Hillcrest High School Broadcast Journalism classes, under the supervision of Mr. Dave Davis.  

I further authorize Mr. Davis to seek any emergency medical attention for my child that he deems necessary during that event.
